Please find below forms that may be required during your student's time at CPS:

• Medication Administration authority.
  • If your child needs medication administered during the day for which you have not previously provided authority (for example antibiotics) you are required to complete the following form and provide a copy plus the medication to the Office. Please note that medication will not be administered without the competion of the form.

If you need to submit one of the following medical plans for your student, please ensure the form is in COLOUR and has an up-to-date colour photo.
